Severe-rated Findings

• 连续工作28天以上 28+ consecutive work days
• 每周工作100小时以上 100+ working hours / week
• 工厂没装火警/疏散警报，包括视觉警示 No fire / evacuation alarm at facility, including visual alarm
• 所有出口都未注明“出口”标志 No specified EXIT signs at all exits

• 出口被堵  Exits are obstructed

• 所有升降电梯未使用时井口开启，且未设立适当的标志 Elevator shafts are open when elevator is not in use and no appropriate signage is in view
• 接线和电气盒处于不安全状态，未安全放置接线 Wiring and electrical boxes are not in safe condition, wiring is not in safely placed
• 未妥善存放化学品 No appropriate chemical storage is followed
• 未提供各种指定的机械安全指南及防护设备，也未对其进行维护 All specified machinery safety guidelines and guards are not provided and maintained

Severe-rated Findings Workbook

• 工厂评估时如果发现两个以上的严重发现点，就会作出零容忍的评分。If 2+ SEVERE findings are found during a facility assessment, a ZT score will be issued.
  
• 工厂必须纠正问题，并接受星巴克的·，所有严重违规发现点每6个月就要重新接受审核确认。Facility must correct, and have validated by Starbucks, ALL SEVERE FINDINGS for a 6 month approval validity
  
• 第三方审核员完成现场评估后，系统信息立即得到更新，还会生成新的核查有效期。Once an on-site assessment is completed by a 3rd party monitor, the system is updated, and an updated approval validity will be generated.
• 只有先纠正过严重问题并接受了星巴克的核查，第三方才会提供整改计划（CAP）管理支持。Corrective Action Plan (CAP) management support from the 3rd party will not begin until the severe issues are corrected and validated by Starbucks.
  
• 如果严重违规发现点涉及工时或休息天数，接受现场审核时必须出示证据，证明问题至少在三个月中都得到过改善。问题如果得到改善，新一轮审核将在12个月后进行。If SEVERE findings are working hours or rest days, a minimum of 3 months demonstrated improvement is required with an on-site assessment to validate.This will result in 12 month validity if improved.
  
• 如果其中7项严重危及健康安全（EHS）的违规发现点当即得到纠正。If SEVERE 7 Health and Safety (EHS) findings are corrected immediately:

• 星巴克作出内部案头审核，有效期为6个月。工厂应在6个月内再次接受评估，确保审核确认的有效性。Validation via Starbucks internal desk review = 6 month validity. Facility should be reassessed within 6 months to ensure approval does not expire.
• 第三方进行现场核查，有效期为1年。Validation via on-site, third party review = 1 year validity.
